*Required Documents for Electronic Visa (Adult) with Insurance

• Visa Application Form : Fully completed and signed by the applicant. Valid Passport : Issued within the last 10 years, Valid for at least 3 months beyond the scheduled return date, Contains a minimum of two blank pages. Photographs : Two recent, high-definition, colour passport-sized photos (not older than 6 months), White background with the face covering 80% of the frame, per ICAO standards. Proof of Means of Transport :Confirmed flight reservations or other transport documents.
• Proof of Lodging : Hotel bookings, rental agreements, or campus residence confirmations, If staying with family/friends : Proof of sponsorship or private accommodation verified by the competent Schengen State authority (if required). Proof of accommodation in each Schengen State being visited (if applicable). Cover Letter : Signed by the applicant, explaining the purpose of the visit and the travel itinerary.
• Travel Insurance : Coverage of at least €30,000 for medical emergencies, accidents, or evacuation, Valid across all Schengen countries, covering the entire travel duration. Proof of Financial Means : Recent private bank statements (last three months), stamped and signed by the issuing bank, Indian income tax return acknowledgments for the past three assessment years. Additional Documents (Based on Applicant's Profile): For Employed Applicants: Last three months' pay slips, Employment contract, Employer's approval for leave. For Company Owners/Self-Employed Individuals: Company registration certificate, GST registration documents.
• For Sponsored Applicants: Proof of sponsorship and/or private accommodation (verified national form) ,Notarized consent letter from the sponsor or guardian (if applicable). For Retired Applicants: Pension statements for the last three months, Proof of regular income from property/business. For Minors: If traveling with one parent: Notarized consent from the non-accompanying parent. If traveling alone: Notarized consent from both parents/guardians, Copies of parents' passports or birth certificate of the minor.
• For Students: Proof of enrolment in school/university, Copy of student ID card. Purpose-specific Documents: Tourism : Travel agency booking confirmation or other travel plans, Marriage certificate (if applicable). Business : Invitation letter from the inviting company/organization, Employer's cover letter confirming identity, purpose, duration, and place of stay. Medical Treatment : Certificate from a doctor or institution in India recommending treatment in the Czech Republic , Confirmation from the receiving medical institution in the Czech Republic. Family/Friend Visits : Invitation from host with address, contact details, and intended stay period , Proof of host's legal residence (passport copy or residence permit). Other (Cultural/Sports/Religious/Film Crews) : Invitation letters, entry tickets, or programs relevant to the event.
