*Required Documents for Electronic Visa (Adult) with Insurance
- Application Form duly filled in and signed by the applicant. One recent passport-sized photograph (3.5 cm by 4.5 cm) in colour with a white background. Passport/Travel Document must: Have a validity of at least 3 months beyond the intended stay. Have been issued within the past 10 years and the validity must not exceed 10 years. Contain at least 2 blank pages. Certificate from travel agency for prepaid programs or reservation of accommodation (with proof of deposit or cancellation fee). Salary slips for the last three months from the employer.
- Detailed bank account statement of the applicant for the last three months. Pension slips for the last three months (if applicable). Certificate of registration for self-employed applicants. For Minors: If traveling with one parent, notarized written consent from the other parent or guardian (unless sole custody is proven with documentation). If traveling alone, notarized written consent from both parents/guardians. Copies of ID documents of parents/guardians. Travel itinerary, including travel dates and return flight reservation or ticket. Overseas travel medical insurance valid in Schengen countries, covering at least €30,000 for emergencies.
