*Required Documents for Electronic Visa (Adult) with Insurance

• Application Form: Fully completed and signed by the applicant. Photograph: One recent passport-size colour photo with a white background, following ICAO specifications. Passport/TraveI Document: Issued within the past 10 years (validity should not exceed 10 years). No handwritten passports, manual observations are not accepted. Minimum validity: 3 months after the intended stay with at least 2 blank pages. Cover Letter: Explain the purpose and details of the visit. Travel Medical Insurance: Coverage of at least €30,000.Valid in all Schengen countries for the entire stay. Must cover costs related to medical repatriation, emergency medical attention, and hospital treatment. Passport Copies: Photocopies of the first and last pages, all visas, and entry/exit stamps.
• Previous Passport: If applicable. Evidence of Employment: If the applicant is an employee: Headed paper company's original approved letter, latest 3 months pay slips and latest 3 years Income Tax Return. If the applicant is Self-Employed: Full Business Certificate of Incorporation + last 3 months Business and Personal Bank Statement + latest 3 years Income Tax Return. If students applying: Original letter from the school, dated within 30 days before date of submission, must report: Full name of the applicant, full name and address of school, telephone number of the school, signature of the Head of Department. Travel Itinerary and Accommodation: Confirmed hotel reservations covering all travel dates. If visiting other Schengen countries, provide hotel bookings for each and Confirmed flight tickets.
• In Case of a Sponsor: Invitation Letter from the sponsor. Copy of the sponsor's passport/identity card. Bank Statements for last 3 months with stamp and sign and ITR for last 3 years For Minors Traveling: If traveling with one parent or third party, parents must sign a Consent form at the Consulate General of Italy, Mumbai (appointment needed).Alternatively, an affidavit with no objection to travel, notarized, legalized, and Apostilled by MEA. For parents residing in Italy, provide a self-declaration with an authenticated signature from their Municipality. Additional Documents: Passport and Visa Copies: Photocopies of the passport (first and last pages), previous Schengen visas, and any valid US/UK visas. Overseas Medical Insurance
