*Required Documents for Electronic Visa (Adult) with Insurance
- Application Form -Duly filled and signed by the applicant. Photograph - One recent passport-size photo in colour with a white background (3.5cm x 4.5cm) , Please do not staple the photograph to the form. Passport/Travel Document -Valid for at least 3 months beyond the intended stay, Issued within the past 10 years and not valid for more than 10 years , Contains at least 3 blank pages. Overseas Travel Medical Insurance - Valid for all Schengen countries with coverage of at least €30,000 , Must cover the entire trip duration (arrival and departure dates) , Only policies from one of the 16 Schengen-recognized insurance companies in India are accepted.
- Flight Reservations - Return or round-trip ticket reservations , If visiting multiple Schengen states, provide proof of intra-Schengen travel (flight, train, or car rental). Travel Agency Confirmation or Trip Plan - Certificate of booking for an organized trip or relevant travel itinerary document. Proof of Accommodation - Confirmed hotel booking or: A 'Declaration of Proof' completed and signed by the invitee, including their Malta ID or passport copy , Supporting documents (proof of residence, income, health insurance, etc.) must be notarized. Proof of Financial Status (Based on applicant's employment status): Employed: Bank statements (last 3 months), signed and stamped , Payslips (last 3 months), employment contract, employer's holiday approval letter , Indian income tax return acknowledgment for the past 2 years.
- Company Owner/Self-Employed : Bank statements (last 3 months), signed and stamped, Company registration and GST certificate , Indian income tax return acknowledgment for the past 2 years. Sponsored: Sponsor's financial documents and proof of accommodation , Applicant's bank statements and income tax returns. Retired: Pension staternents or proof of regular property/business income.
- Minors: Notarized arental consent if traveling with one parent or alone. Parent(s)Pguardian(s) ID copies. • Original Passport Bio Paae + Photocopy • Aadhaar Card (Originar+ Photocopy)
